We have trained and practicing RNs & ENs who are APHRA registered to provide nursing supports to participants in the community. Our RN's can provide incontinence assessments for your clients and provide a report.

We can tailor Respite with activities which we can tailor to participants likes like fishing either by the jetty or on a boat, hiking and orienteering. Call us to discuss your participants ideal respite and we will work with you, your participant or your loved ones to make memorable experiences.

PSCC provides personalised catered support for day-to-day access for social, community and in home care. We hand pick suitable support workers that fit each participants specific needs. Our goal is to assist participants to live enriched and independent living.

NDIS participants can have a service agreement arranged to assist them in accessing Support Services. Fill out the Participant Referral Form to get in contact with us today.

Funding through the NDIS for transport assistance is possible if funded in the plan. If the participant cannot use public transport without substantial difficulty due to their disability.

PSCC carers are able to transport the participant with a disability for everyday commitments, appointments and community access events.

PSCC staff will assist with daily health and hygiene needs, helping to create daily routines and habits that will help to increase the independence and capabilities of each individual participant. Such as: Showering, Hair and body care, General personal Hygiene etc.

This also includes needs such as: Stoma care, Bowel Care, Wound care (Nurse Assistance (RN / EN) as required) Medication administration/charts and all documentation /record keeping.

Domestic assistance can include tasks such as household cleaning and laundry, lawn or garden care, whilst supporting the participant in engaging in these daily activities helping to create functional and practical structure to their daily routines.

At PSCC we understand that maintaining your yard can be difficult. We want to make sure you have an outdoor space that is functional and that you can be proud of. We have support workers to assist and maintain your garden by mowing, gutter cleaning, roof repair and anything in between.

If you have Domestic or Personal Assistance in your NDIS plan or My Aged Care plan, we can access this funding to help maintain your yard to pristine condition by maintaining:

Lawn Care:

Lawn care can be difficult at the best of time! Let our staff assist you with mowing, whipper snipping, edging and weeding.

Gardening:

Whether you need assisting planning and planting a new garden or trimming back overgrown hedges, we are available to help assist you in the garden. Let us take out the trash for you! We also help with rubbish removal, yard clippings, household garbage and clutter.

SIL provides around the clock (24 hour) day-to-day support for those wanting to live independently but have a higher set of needs. SIL Housing can be provided at a 1:1, 1:2 and 1:3 ratio.

PSCC offer 24hrs support in SIL, STA, MTA housing. Depending on the needs of each participant, we can arrane this as directed by the nominee or participants representative. Housing - Household goods - 24hrs in home and community support workers - Transport - Activities and more.

STA and Respite, is funding for support and accommodation for a short time away from your usual home. It covers the cost of your care, accommodation, transport, food and daily activities in another place for up to 14 days at a time. You might have a short stay with other people, or by yourself. It's often funded when your usual carers aren't available, or for you to try new things.

PSCC and its staff help participants to access social and community events such as group activities and independently, and externally Activities such as: Art based, Music, Food, Creative, Markets/stalls and much more. Each participant can decide on what events they would like to partake in and our staff will investigate as per the participants' likes and interests. Transport to and from all allied health appointments and daily routine needs.
